Changes to your enrollment can be made through BENEFEDS if you have a Qualifying Life Event (QLE) or during Open Season. Each year, open season runs from the Monday of the second full work week in November through the Monday of the second full work week in December. Any changes made due to a QLE will be effective as of the next premium billing cycle. Any changes made during Open Season will take effect as of the next January 1.
Is my family covered?
When you enroll in FEP BlueVision, you have a choice between three enrollment types, two of which allow you to add eligible family members to your coverage. You can check your family’s eligibility by logging into your member account.
How do I access information online?

- This brings you to the FEP BlueVision benefits main menu.
- You do not need to be registered to access this area of the website.

- If you have already registered with us, enter your user name and password.

What information will my eye doctor need?
When scheduling your appointment with an in-network provider, it’s best to have your member number so they can quickly verify eligibility. You may also use the policyholder’s name and the state in which they live.
Where do I find my member number?
Your member number can be found on your Member ID Card or by calling us at 1-888-550-2583.
Do I get a member ID card? What if I lose it?
All FEP BlueVision identification cards show only the policyholder’s name. When your covered dependent visits an FEP BlueVision participating provider for routine eye care services, the provider will use the identification number from the card as well as the patient’s name and date of birth to verify eligibility and obtain authorization to provide benefits.
Please note that FEP BlueVision identification cards are sent when initially join with us and as requested. Requests may be submitted by email or telephone.
What if my provider is not in-network?
FEP BlueVision High Option Members have the option to use your benefit with an FEP BlueVision participating provider or visit a provider of your choice, pay in full, and file a claim for reimbursement of up to the scheduled maximum. If you receive routine eye care services outside of your network of FEP BlueVision participating providers, please fill out the Direct Reimbursement Claim form completely and send it to the address on the form. A copy of your paid receipt can take the place of the provider’s signature.
TIP: Bring the Direct Reimbursement Claim form with you to your appointment for easy completion of provider information and for provider’s signature.
FEP BlueVision Standard Option members must receive routine eye care services with an FEP BlueVision participating provider, who will apply the benefit at the time of the visit. There is no out of network reimbursement available with the FEP BlueVision Standard Option, except under certain circumstances.
What is included in an eye examination?
The routine eye examination covered by your FEP BlueVision benefit yields a basic prescription for eyeglasses, and includes a refraction, basic glaucoma (puff-of-air) test, Dilated Retinal Examination (if professionally indicated), and a general overview of eye health. This examination can detect a number of eye diseases, as well as signs of systemic conditions such as diabetes, thyroid disease, high blood pressure and neurological impairments. Every eye examination our providers administer is consistent with clinical guidelines published by the American Optometric Association and the American Academy of Ophthalmology.
What is a dilated retinal exam?
A Dilated Retinal Examination (DRE) is a critical diagnostic procedure in the detection and management of diabetes, glaucoma, hypertension and many other ocular and/or systemic diseases (up to 30 altogether!). It can lead to higher quality patient care, improved lifestyle through early detection and intervention, and possibly lower your overall health care costs.
What options do I have for laser vision correction services?
The laser vision correction benefit allows members of FEP BlueVision to realize savings off the national average cost. FEP BlueVision has partnered with QualSight, whose laser providers are credentialed according to NCQA standards and represent ophthalmologists and surgeons who use the latest, most advanced instrumentation. The discount program is applicable to Lasik and PRK. When you receive Lasik surgery services with a participating provider, they will apply your benefit at the time of your visit. There is no out of network reimbursement available for Lasik surgery with FEP BlueVision.
To locate a participating laser vision correction provider and learn how to schedule your pre-operative evaluation, please call QualSight at 1-855-502-2020.
What is the Exclusive Collection?
The Exclusive Collection is a unique offering to FEP BlueVision members only and can be found in nearly 9,000 provider offices across the country. Just log in to your online member account, click on “Find a Provider” and look for the glasses icon.
FRAMES: A collection of over 200 frames available to members at no cost. To browse frames, click “Personal Frame Collection.”
Do I have to get all services completed at the same time?
As an FEP BlueVision Member, you can get a routine eye examination and shop for eyewear at different times and in different locations. For example, you may get your eye examination from an eye care provider and then use your prescription to shop for glasses at an in-network retailer at a later date within the same plan year.
Does my whole family have to visit the same provider?
No. Dependents can visit different providers. For example, students who are away at school can locate a participating provider near them.
What if my glasses break?
All frames purchased in-network and crafted in our laboratory come with a one year breakage warranty. If your glasses break, simply return them to where you purchased them – we’ve got you covered.
Can I use my insurance when shopping online?
You may use your in-network benefit with www.visionworks.com. There are no other online suppliers of eyeglasses or contact lenses who are contracted with FEP BlueVision at this time.
